RENEWAL OF ACCREDITATION

UPDATED VENDOR FORM (June 9, 2017): FM-GBP-MMD-PO-017 Vendor Accreditation Update Form

This is to remind all accredited vendors of Global Business Power of the submission of the hard copy of the following documents not later than 30 June 2017:

  • Accomplished Vendor Accreditation Update Form (link)
  • 2016 Income Tax Return (must be duly stamped by BIR)
  • 2016 Audited Financial Statement (must be duly stamped by BIR)
  • 2017 Business Permit/Mayor’s permit
  • Updated DTI Registration Certificate (for Proprietorship only)
  • Updated Distributorship Agreements

Please be advised that the Accreditation of suppliers who failed to submit the required documents on the deadline set will be put on Conditional Suspension.

APPLICATION FOR ACCREDITATION (FOR NEW SUPPLIERS)

  1. Supplier must accomplish the official Accreditation Application Form in full and sign as required (Note: Signatory’s position must be any of the following President, CEO, General Manager, Owner, Proprietor. Otherwise, please provide Secretary’s Certificate authorizing the signatory)
  2. All applicable required documents must be submitted and complied with.  Please take note of the additional requirements per industry specific i.e. coal, contractors, OEMs, traders, etc.  The list of required documents is attached for reference and guidance.
  3. For initial assessment, required documents should be saved in a CD and send by courier together with the application form to:

  4. Once received, documents will be reviewed for accuracy and completeness, Supplier will be given an appointment/schedule after the initial review. Supplier must bring 1 set original and 1 set photocopy on the scheduled appointment.
  5. From the documents submitted, technical, financial capabilities and performance record will be evaluated and validated.  Once your accreditation is approved, you shall be officially advised of your accreditation.
  6. Unless we communicate, initial follow-up will be entertained after ONE (1) month from acceptance of documents.
